The Science of Elaborative Interrogation: Ask 'Why' to Learn More

One of the most powerful and underutilized study techniques in FNP board preparation is elaborative interrogation — the practice of asking "why" rather than just "what" when studying new content. Instead of simply reading that metformin is the first-line treatment for type 2 diabetes, elaborative interrogation asks: why is metformin first-line? What is its mechanism? Why does that mechanism make it preferable to other agents? What are the clinical scenarios where it would not be first-line, and why?

This approach — systematically generating explanations for the facts you are learning — produces significantly better retention and better transfer than passive reading or simple memorization.

The Research on Elaborative Interrogation

The research on elaborative interrogation is robust. A meta-analysis by Dunlosky et al. (2013) — one of the most comprehensive reviews of study technique effectiveness ever conducted — rated elaborative interrogation as a "moderate utility" technique, significantly more effective than re-reading, highlighting, or summarizing. The mechanism is straightforward: generating an explanation for a fact requires you to connect it to existing knowledge, and those connections are what make the memory durable and retrievable.

How to Apply Elaborative Interrogation to Board Prep

The most direct application is to add a "why" question to every fact you study. When you encounter a clinical guideline, ask why it exists — what is the pathophysiology that makes this the right recommendation? When you encounter a drug, ask why it works — what is the mechanism, and what does that mechanism mean for the clinical scenarios where it is and is not appropriate? When you encounter a diagnostic criterion, ask why that threshold was chosen — what is the evidence, and what are the clinical implications?

This approach takes more time than passive reading — but it produces learning that is far more durable and far more applicable to the clinical reasoning that board questions require.
