Navigating Psychiatry on the FNP Boards
Psychiatric questions make up a substantial portion of both the AANP and ANCC family nurse practitioner certification exams. While the volume of diagnostic criteria can feel intimidating, the board exams consistently test a core set of predictable concepts. If you understand safe pharmacological management, accurate diagnostic timelines, and critical safety red flags, you can conquer this section with complete confidence.
1. Selective Serotonin Reuptake Inhibitors (SSRIs) and Safety First
SSRIs remain the first-line pharmacotherapy for major depressive disorder, generalized anxiety disorder, and panic disorder. However, board exams love to test your knowledge of adverse effects and safety warnings.
When initiating an SSRI, always remember that therapeutic effects take 4 to 6 weeks to fully manifest. Patients often experience transient side effects—such as nausea, headache, and insomnia—within the first few days, but these typically resolve.
- Clinical Pearl: Always screen patients for a history of mania before prescribing an antidepressant. Unmasking underlying bipolar disorder with an SSRI can precipitate a manic episode.
- Board Tip: Watch out for Serotonin Syndrome, characterized by altered mental status, hyperreflexia, myoclonus, and autonomic instability. It occurs most commonly when two or more serotonergic agents (like an SSRI and tramadol or MAOI) are combined.
2. Major Depressive Disorder vs. Normal Grief
Distinguishing clinical pathology from normal human experiences is a cornerstone of primary care testing. Major Depressive Disorder (MDD) requires at least 5 of 9 symptoms present during the same 2-week period, and at least one of the symptoms must be depressed mood or loss of interest/pleasure (anhedonia).
Remember the mnemonic SIGECAPS: Sleep changes, Interest loss, Guilt/worthlessness, Energy loss, Concentration difficulties, Appetite changes, Psychomotor agitation/retardation, and Suicidal ideation.
- Clinical Pearl: In normal bereavement, self-esteem is generally preserved, and feelings of emptiness or loss come in waves, often associated with thoughts of the deceased. In MDD, depressed mood is persistent, accompanied by pervasive self-loathing, worthlessness, and a fixation on ending one's life.
3. Bipolar Disorder: Protecting Against Antidepressant Monotherapy
Diagnosing and managing bipolar disorder requires a delicate hand. The hallmark of Bipolar I disorder is the presence of at least one manic episode, which may be preceded or followed by hypomanic or major depressive episodes. Mania is characterized by inflated self-esteem, decreased need for sleep, grandiosity, pressuring speech, and risk-taking behaviors lasting at least 7 days.
- Board Tip: Never prescribe an SSRI as monotherapy to a patient with bipolar disorder. Doing so can trigger rapid cycling or a severe manic switch. Mood stabilizers, such as lithium or lamotrigine, form the foundation of treatment.
4. Screening Tools and Suicide Risk Assessment
Primary care providers are often the first line of defense for mental health screening. Expect board questions asking for the appropriate screening tool for a given clinical scenario.
- PHQ-9: The gold standard for screening and monitoring the severity of depression.
- GAD-7: The primary tool for assessing generalized anxiety disorder.
- CAGE or AUDIT-C: Essential for evaluating alcohol use disorder.
Whenever a patient endorses depressive symptoms or expresses hopelessness, your immediate clinical priority is to assess for suicidal ideation with a plan and intent. Never hesitate to ask direct questions about suicide; research consistently shows that asking does not increase risk.
